New Hampshire, Maine or Vermont

If you have business services in New Hampshire, Vermont, or Maine that you wish to disconnect, use this form.

If you have business services in any other state or location that you wish to disconnect, use this form.

Before you disconnect

A few things to keep in mind when disconnecting business services.

Equipment returns

Any Consolidated-owned equipment must be returned. We'll provide instructions and shipping labels after your disconnect request is processed.

Final billing

Your final bill will be generated after your services are disconnected. Any remaining balance will be due according to your normal billing terms.

Contract terms

If your services are under a contract, early termination fees may apply. Please review your service agreement or contact us for details.

Back up your data before disconnecting

If you use a Consolidated email address or web hosting services, please back up your data before disconnecting. Email accounts and hosted content will be removed after your service is disconnected.
